Hi all - sorry to be a pain but it has been brought to my attention that folks with older Macs cannot open the Excel spreadsheets I sent out previously, which were in xlsx format. I have saved them down to an Excel 97-2003 workbook...hope this helps.The attached forms are in xls format and should allow those folks to enter their data. If you can use the first set of forms, I would prefer that you use them however!
