For those who don't have easy access to the New England Jounal of Medicine,
the ALDF has put the recommended treatment table on their web page:

The ALDF really has a lot of useful information on their site, and links to
other websites with some of the differing opinions I discussed earlier -
including a link to QuackWatch, a valuable site that explores the validity
of some alternative treatments. The ALDF's main site is:

Doxycycline is one of the tetracycline class of antibiotics. One of the main
side-effects of concern to the birding community is that it increases
sensitivity to sunlight and the risk of sunburn.

Amoxicillin is in the penicillin family and would not be usable in people
with a penicillin allergy.

I don't know anything about the other antibiotics listed in the table, and
don't know why doxycycline is the particular type of tetracycline usually
used - as I said, I'm not a doctor (either MD or PhD...I dropped out of grad
school!)
