me, then you know I received your entries.  If you sent some to me and
didn't get that "Thank you", then get in touch with me NOW.

 

If you haven't submitted yet, you have until cob Wednesday, 28 January.  Any
I receive after that date probably won't make it into the List Report.

To use this form without error you must have either Adobe Acrobat or Adobe
Reader, versions 8 or 9.  Earlier versions just won't work with the form.
This shouldn't be a big burden since Adobe Reader is free at the Adobe
