I have the List Report Form ready now for your use.  Taylor Maclean has a
copy to put up on the MOS website and I have placed a copy on the MDOsprey
website, as well.

 

Go to www.mdosprey.org and click on the 2009 List Reporting Form hotlink.

 

You will see a pdf form that you can fill out online and then print or send
to me via e-mail.  E-mail is much easier for both of us but if that won't
work for you, print the form and mail it to me via snail mail at the address
on the form.

 

I'm looking forward to hearing from you this year!  Submissions due by the
end of January 2010.
